A coroner's report into a fatal incident at a Metchosin construction site finds incorrect equipment contributed to a death an expert has described as preventable.
The house construction project was being completed by Westwater Developments Limited. Forty-two year-old Robert Kenneth Davison was an electrician working for sub-contractor VI Electric Limited.
Davison was killed on August 13 of last year when a rough terrain forklift being operated by a Westwater employee tipped while hoisting a utility pole, and the pole fell on Davison's head.
An expert contracted by the RCMP to review the use of the specific kind of forklift used that day found the machine was not designed to erect utility poles.
An assessment of site safety found multiple violations tied to safe work procedures, training, and equipment maintenance and records.
WorkSafe BC conducted an investigation, but it's findings have not yet been released.
